The form below initiates the medical waste service between Advant-Edge and UPenn for medical waste services. You will need the following items:
- PO number and amount.
- Floor, room and expected pickup frequency info for each location on the PO.
- If a room on your PO is a shared space, please be specific in the notes for describing the location/lab.
- The email address to send manifests related to this PO, this will serve as a service contact as well.
